Safeguarding Adult Review in Respect of ‘James’

The North Yorkshire Safeguarding Adults Board and North Yorkshire Safeguarding Children Partnership have accepted the findings and recommendations of a Safeguarding Adult Review (SAR) that the Board commissioned regarding ’James’, who sadly died in December 2019.

James was an 18-year-old man whose main contact with the former North Yorkshire County Council was with the children’s and young people’s services and became known to adults’ services shortly before his death.

As James died as an adult, a Safeguarding Adult Review was undertaken. However, the review was undertaken jointly by the North Yorkshire Safeguarding Adults Board and North Yorkshire Safeguarding Children’s Partnership to reflect the involvement both services had with James.

The purpose of a Safeguarding Adult Review (SAR) is to look at how multiple agencies can best work together to protect people and provide recommendations on how lessons can be learned from their experiences to improve practice and services.

The report makes a number of recommendations to the individual agencies involved and the North Yorkshire Safeguarding Adults Board and North Yorkshire Safeguarding Children’s Partnership as a whole, all of which have been accepted in full.

Both the North Yorkshire Safeguarding Adults Board and North Yorkshire Safeguarding Children’s Partnership express their sincere condolences to James’ family and friends for their loss.

An executive summary of the report can be viewed below.
